Work Performed:
  • This position reports to the Supervisor of Social Services.  The Intensive Community Outreach Coordinator provides specialized and intensive support to vulnerable individuals and families facing homelessness and complex barriers to housing stability.
  • The expected hours of work for this position are 8:30 am - 4:30 pm.
  • Conducts client interviews (onsite in office/offsite in client homes or in community setting)
  • Completes initial assessment, schedule and organize follow-up appointments and services
  • Lead and facilitate case conferences with internal staff and other professionals as required
  • Obtains, create and maintain client documentation and records
  • Provides case management services for multiple clients
  • Delivers other required individual or group life skills programs or services, including coordination and facilitation of circles of support with other helping professionals to ensure clients are adequately and appropriately served
  • Attends community and stakeholder meetings and other inter-professional care­ coordination meetings as required
  • Designs and delivers life skills and psycho-educational programming for clients
  • Co-ordinates training for other human service professionals on issues pertaining to homelessness and housing stability
  • Acts as a resource to internal staff and other affiliated agency staff to foster housing stability and retention for individuals and families
  • Reviews research and makes recommendations to management team members regarding program development, implementation, and evaluation
  • Lead collaborative efforts within the Perth Stratford Housing Corporation to maintain housing stability and ensure successful tenancies
  • Lead the outreach team’s approach to and interaction with high-acuity individuals and families experiencing homelessness
  • Host and facilitate community education sessions on City of Stratford programming, response models, and community inquiries
  • Act as lead for community response to unsheltered individuals, including dispatching resources, determining level of need, and coordinating responses to public information requests
  • Coordinate community events and support programming, including in-building support programs, with community partners
  • Work with emergency services in community settings and public housing buildings to assess and respond to complex situations involving clients
  • Represent City of Stratford on Situation Table
  • Assist in maintaining a safe work environment
  • Perform other related duties as assigned

Qualifications:
  • 3 year diploma in applied Social or Health Sciences, or
  • 2 year diploma in applied social or health sciences plus 1 year additional in specialized training social or human services
  • Minimum 1-2 years related professional experience
  • Valid Driver's Licence (Class G) - Travel throughout service area will be required
  • Successful Criminal Reference Check (vulnerable sector)

Specialized Skills, Abilities, Training, License, Memberships, or Certifications:
  • Completion of coursework or training (workshops, conference attendance, continuing education courses) in mental health, addictions, concurrent disorders, trauma-informed practice, motivational interviewing and stages of changes, recovery-oriented case management, community development, screening and assessment (Vulnerability Index, Service Prioritization Decision Assistance Tool), client record keeping and professional ethics, group facilitation, adult education.
  • Strong written and verbal communication skills
  • Knowledge of HIFIS, VI-SPDAT, YARDI and SAMS preferred
  • Demonstrated knowledge of social and community resources
  • Ability to manage time and resources to complete a range of tasks and activities to a high level of consistency and accountability
