Job Summary
The Integrated System Navigation service is a holistic approach to providing integrated human services in the Social and Community Services department. Reporting to the Supervisor of Program Integrity, Human Services Planning and Program Support, the Integrated System Navigator supports individual and families by creating a client-led service plan including the identification of specific service goals to improve life stability and enhance well-being including: 
  • increased client access to core programs,
  • enhanced coordination of supports internally and externally in the community,
  • increased access to eligible income supports,
  • increased housing stability,
  • increased labour attachment (where possible)
  • increased access to early learning and child care, and special needs services, and
  • improved life stability and well-being

Duties & Responsibilities
  • Foster quality service and ensure continuity of service to individuals and families with complex needs by creating a non-judgmental and non–punitive environment.
  • Actively engage with individuals and families to facilitate a client-led action plan using a strengths base approach.
  • Initiate and assist client/family in completing applications, forms, and documentation to explore potential additional supports and financial resources.
  • Complete documentation related to client assessments, decision-making, and discussions in a timely and accurate manner.
  • Consistently working with a client/family to move towards self-sufficiency and improved life stability. 
  • Communicate with front-line staff involved with client/family to ensure consistency in program delivery and avoid duplication of service using a coordinated and collaborative approach to problem-solving.
  • Provide support to staff and Human Services Providers through consultation, case conferencing, and information/training related to available community supports and services.
  • Provide follow-up to requests for support for individuals/families from Access Halton, the Commissioner of Social and Community Services, the CAO’s office, and from local constituents’ offices.
  • Perform other duties as assigned.

Skills & Qualifications
Essential: 
  • A Bachelor’s Degree in Behavioral or Social Sciences or related field or approved equivalent combination of education and experience.
  • Minimum five (5) years’ experience working in the human services field with experience in case management, trauma-informed care and experience working with individuals/families with complex needs e.g. mental health, addiction, homelessness, required.
  • Strong understanding of cultural safety and cultural competency for Indigenous/racialized populations.
  • Demonstrated knowledge of community supports and services.
  • Knowledge of the Department’s program delivery operations, programs and services, the communities served, and client/population social issues.
  • Proven ability to build and maintain positive professional working relationships with internal and external partners.
  • Ability to develop and recommend creative solutions, with the ability to adapt to changing environments, and work both independently and in a team environment.
  • Advanced problem solving, active listening and oral and written communication skills.
  • Must be a team player and able to work collaboratively with and through others.
  • Proficiency working with Microsoft Word, Excel and PowerPoint

Preferred: 
  • Broad knowledge of legislation and/or the ability to interpret legislation under the Ontario Works Act, Ontario Disability Support Program Act, Social Housing Reform Act, Child Care and Early Years Act.
  • Working knowledge of SAMS/SALI, OCCMS and Yardi is preferred.

Working/ Employment Conditions
Working Conditions: 
  • Travel may be required within Halton Region, incumbent must provide their own transportation. 
  • Requirement to work in person and in the community with individuals and families.

Employment Conditions:
  • The incumbent will be required to provide a copy of their driver’s license by first day of employment.
  • A current (obtained within the past six (6) months), original and acceptable Criminal Records Check, including Vulnerable Sector Screening.
